3 STEPS ON HOW TO REGISTER WITH DINSDALE MEDICAL CENRE

Complete the enrolment forms in our clinic, or click the PDFs below to download our forms and email them to admin@dinsdalemed.co.nz

Provide a copy of your
photo ID showing NZ residency status such as:
- Passport
- NZ Birth Cert & Driver Licence
- Passport & Resident Visa
- Passport & 2+ Year Working Visa
3.
Wait for reply from our team and Pay the $10 enrolment fee into our bank account:12 3123 0025770 00 with your Full name as Reference.

*PLEASE DO NOT MAKE PAYMENT BEFORE YOU HEAR FROM OUR TEAM.
After you have completed the enrolment forms, provided ID and paid the enrolment fee we can request for your medical records to be transferred to our clinic.
This process normally takes between 5 -10 working days.

Once we have received your medical records then we can book you an appointment to see a doctor.
